2011 Registration Information

6-8pm @ Staley Upper Elementary School Auditorium

LATE SIGN UPS (Cost increases)
Wednesday, June 8th & Thursday, June 9th
Flag & Flag Cheerleading Cost: $60 for 1st participant, $40 for each additional child Tackle Football & Cheerleading Cost: $85 for 1st participant, $50 for each additional child

Be sure to bring the child(ren) that are being registered to the sign ups. (They need to sign the Contact and football players need to be weighed.)

Registration Fees? See above. The cost depends on the sign up date. There is a manditory $10.00 Concession Stand Fee that is reimbursed when the parent works in the Concession Stand. There is also a Manditory fundraiser at Applebees for a pancake breakfast on Sunday, August 31st. Each family must purchase 4-$5 tickets ($20.00) to attend or sell.

Who is eligible?Tackle Football: Boys & Girls ages 7-13 as of July 31, 2011. Cheerleading: Boys & Girls ages 5-13 as of July 31, 2011. Flag Football (Non-contact): Boys & Girls ages 5-7 as of July 31, 2011.

Forms Needed at Registration:

Please Print out the documents below and bring them to Registration. Also, bring a copy of the childs Birth Certificate.

Medical Form 2011 (the second page needs to be completed by your doctor) and Participant Contract and Parental Consent Form 2011

(These forms will also be available at registration.)

*Required: ALL players and cheerleaders need to provide a copy (all pages) their most recent Report Cards to the league. This is so that our children can participate in the National Little Scholars Program.

Refunds- Refunds will not be given after August 1st.
